Medical History Program
In cooperation with the Aging Well Community Action Team of La Grange, the La Grange Fire Department participates in the File Of Life program.

A File of Life is a mini medical history in a packet attached with a magnet to the outside of a refrigerator. Our firefighters / paramedics are trained to look for the packets in this location. Paramedics will use this information while transporting you to the hospital and will provide it to hospital personnel.

Information for the Packet
Patient information to put on the File of Life card includes:

Allergies Emergency contact Health care proxy location

Health problems Insurance policy Medications and dosages

Name Recent surgeries Social Security Number (optional)

This information should be duplicated onto the wallet-sized form to be carried by the patient. Write in pencil so updates can be made easily. You should update both forms twice a year as needed.
